Chikungunya: Joint Pain, Symptoms, Tests and Recovery Guide

Chikungunya is a mosquito-borne viral fever known for severe joint pain that can last weeks or months. Rest, fluids and paracetamol help, and dengue should be ruled out before using other painkillers.

What is chikungunya?

Chikungunya is a viral infection spread by the same Aedes mosquitoes that spread dengue. These mosquitoes breed in clean stagnant water around homes and bite during the day. Outbreaks occur in India during and after the monsoon, and Karnataka and Andhra Pradesh have both seen large outbreaks in the past.

The name comes from an African word meaning "that which bends up", describing the stooped posture of people with severe joint pain. It does not spread directly from person to person; a mosquito must bite an infected person first and then bite someone else.

Symptoms of chikungunya

Symptoms usually begin three to seven days after the mosquito bite. The fever usually lasts a few days, but the joint pain can be very strong and may limit walking, gripping or getting out of bed. The pain often affects both sides of the body at once, for example both wrists or both ankles.

  • Severe pain and swelling in joints, often hands, wrists, ankles and knees

  • Sometimes red eyes or nausea

Chikungunya or dengue?

Both start with sudden fever and body pain, and both are common in the same season. Chikungunya tends to cause more intense joint pain and swelling, while dengue more often causes a low platelet count and a risk of bleeding. It is possible to have both at the same time.

Because the treatment advice differs, especially about painkillers, it is important to see a doctor and get tested rather than guessing which illness you have. Until dengue is ruled out, treat the fever as if it could be dengue and use only paracetamol for pain.

Tests

In the first week, an RT-PCR test on blood can detect the chikungunya virus. After about five days, a chikungunya IgM antibody test becomes positive. Your doctor may also order a dengue test and a complete blood count to rule out dengue and check platelets.

If joint pain lasts for months, the doctor may check tests such as ESR, CRP and rheumatoid factor to look for ongoing inflammation or other types of arthritis. These results help decide whether you need a specialist's opinion or longer treatment.

Treatment and home care

There is no specific antiviral medicine for chikungunya. Rest, drink plenty of fluids and eat nutritious food. Paracetamol is the safest first choice for fever and pain; follow the label or your pharmacist's advice. Cold packs wrapped in a cloth may ease swollen joints. Avoid tight rings, bangles or footwear while fingers and ankles are swollen, and rest the joints during the fever.

Painkillers like ibuprofen or other anti-inflammatory medicines should be used only after a doctor has ruled out dengue, because of the bleeding risk. Newborns whose mothers had chikungunya near delivery, elderly people and those with diabetes, heart or kidney disease need closer medical care.

Long-lasting joint pain

Most people recover within one to two weeks, but some, especially older adults, have joint pain and stiffness for months. Gentle movement, stretching and physiotherapy help keep joints flexible. Avoid heavy strain on painful joints but do not stay completely still.

For persistent pain, a doctor may prescribe anti-inflammatory medicines, and in some cases other medicines used for arthritis. Steroids or these arthritis medicines are given only on a doctor's prescription; do not start or stop them on your own. Avoid buying painkiller injections or loose tablets without a prescription, as some contain steroids that can cause harm.

Preventing chikungunya

There is no widely available vaccine in India yet, so preventing mosquito bites is key. Empty and cover water containers every week, clear stagnant water from terraces, coolers and pots, and use repellents, full-sleeved clothes and screens. A person with chikungunya should rest under a bed net in the first week to avoid spreading the virus through mosquitoes.

Community efforts matter too, because Aedes mosquitoes breed in shared spaces such as terraces, construction sites and drains. Encourage neighbours and apartment associations to check these places every week. If you develop fever with joint pain, see a doctor early. Your doctor or pharmacist can advise on safe pain relief.
